I am the country lead on the most comprehensive epidemiologic study on the planet- the Global Burden of Disease (GBD) Study since 2016 contributing to 60+ high impact GBD-related peer-reviewed publications, and being one of the most cited Irish researchers since 2019 (>85,000 citations since 2019). My two core research areas are: tobacco control and alcohol policy research- advancing the discipline of population health science. In recognition of my 20+ years population health research experience, the Irish Minister for Health appointed me as a member of the newly established Health Research Consent Declaration Committee (HRCDC) in 2019 for 6 years. I am the founding Editor-in-Chief of the Open Access Sage journal- Tobacco Use Insights.
Medical Education and MSc in Public Health (1990-2000): I am a medical graduate from India (MBBS) in 1990, obtained Higher Specialist Training in Public Health Medicine in India (1993-1999) – Doctor of Medicine (MD) in 1996 and Senior Residency (1996-1999) in Community Medicine at the top medical institute in India- the All India Institute of Medical Sciences (AIIMS), New Delhi. In 1999, I was awarded the prestigious Irish Foreign Affairs Aid Fellowship to pursue MSc in Public Health at the highest ranking University on the island of Ireland- Trinity College Dublin (TCD). My MSc publications contributed to W.H.O’s policy on non-specific effect of routine childhood vaccination that led to an invitation as Technical Advisor at W.H.O in 2004.
Doctoral (PhD) journey (2001-2004): In 2001, I was awarded a Trinity Post Graduate Scholarship to pursue PhD at TCD evaluating the impact of the 1990 ban on the sale of bituminous coal in Dublin on lung cancer mortality for the first time under the mentorship of Professor Luke Clancy- an internationally renowned tobacco control researcher and advocate. A section of my broad PhD work featured in the Sunday Times UK in 2006. My PhD project led to my next journey on Non-Communicable Disease (NCD) epidemiology.
Post-Doctoral Fellowship (2004-2006) on CHD IMPACT Modelling Study and its Research Impact: I embarked on a post-doctoral fellowship (2004-2006) in TCD to work with Professor Kathleen Bennett. I co-led an Irish Heart Foundation Award during this period, which was pivotal in the development and validation of the Irish CHD epidemiologic IMPACT model in collaboration with colleagues at University of Liverpool (Professors Simon Capewell and Martin O’ Flaherty). This Irish CHD IMPACT modelling study was instrumental in informing the cardiovascular health strategy (2010-2019) in Ireland, and later the Healthy Ireland Framework. I contributed to the National Heartwatch Clinical Report in 2006 drawn on the IMPACT modelling work. I continued leading on several iterations of the Irish IMPACT modelling studies generating evidence on CVD prevention and future trends in CVD trends, including cost-effectiveness of primary prevention over secondary prevention of CHD in Ireland. I contributed to the Euro Heart II Project 2020 work package 6 (EU-funded) on the future trends in CVD mortality across 9 EU countries, including Ireland. Work from this IMPACT model- a unique policy-focused dynamic scalable epidemiologic model, featured in Irish policy documents on CVD prevention in 2023, in WHO report in 2014, in OECD report in 2015, and informed the HSE National Clinical Health & Wellbeing team on defining the problem of Chronic Disease Prevention in Ireland, while the food policy modelling work on saving 1000 lives in Ireland featured in several Irish media, Work from this Irish IMPACT model is still continuing to have an impact on policy-making – the recent contribution being the Irish Heart Foundation’s position paper on “Primary Prevention of Cardiovascular Disease” in 2023 led by Professor Ivan Perry of the School of Public Health, UCC.
Senior Post-Doctoral Fellowship (2006-2012) on Tobacco Control and its Research Impact:
I was the first recipient of the newly launched HRB-NCI (National Cancer Institute, US) Joint Research Cancer Fellowship to pursue post-doctoral research at Harvard School of Public Health (2006-2008), as an affiliate of the TobaccoFree Research Institute of Ireland under the mentorship of Professor Gregory Connolly and Professor Howard Koh (the former Assistant Health Secretary under President Obama) at the Center for Global Tobacco Control. In this Cancer Research Fellowship, I examined the impact of Massachusetts Tobacco Control Program to inform the Irish tobacco control policy. This Harvard fellowship led to two successful grants- HRB Career Development Award (2009-2012) as PI, and the US FAMRI Award (2010-2013) as co-PI. In recognition of my work on tobacco control, I was appointed Visiting Scientist at Harvard for five years (2008-2013). This senior post-doctoral fellowship led to policy-informed impactful peer-reviewed publications and co-led a policy report on Tobacco Free Future- North/South), which was launched by the former Irish Minister for Health- Dr James O’ Reilly in Belfast in 2013 in collaboration with the Institute of Public Health (IPH).
During this 6-year Fellowship, I comprehensively evaluated the health benefits of the Irish smoking ban, which was cited in the US Surgeon General 2014 Report and in World Health Statistics 2017 Report. My Fellowship also contributed to generating evidence informing tobacco control in vulnerable populations in Ireland, such as gay and lesbian population, ethnic minorities, migrants, children/adolescents, and pregnant women, and more recently among prison population. I was the first scientist in Ireland to quantify secondhand smoke exposure levels to children in private vehicles (2009), which was pivotal to the Irish legislation on smoking ban in cars in 2016. I contributed to an Indian Government Ministry for Health report, as an external peer reviewer on “Smokeless Tobacco and Public Health in India” published in 2016. My two HRB-funded projects were showcased in the HRB Picture of Health publications in 2009 and in 2013, respectively. I was awarded the prestigious Fellowship of the American College of Chest Physicians (FCCP) in 2013 in recognition of excellence and sustainable contributions to the science and advocacy of tobacco control internationally.
My Recent Research Contributions to the Global Burden of Disease (GBD) Study and its Impact: I am the country lead of the Global Burden of Disease (GBD) Study since 2016 and have been trained in GBD methodology at the international GBD training program in Athens, Greece in 2017- sponsored by the UCC College of Medicine & Health. I was invited to serve as a member on the W.H.O. European Burden of Disease Network (EBoDN) in 2018. I have established an Irish Burden of Disease (I-BoD) Research Group towards capacity building (this includes master’s students who have been trained in Burden of Disease (BoD) methodology, and Irish colleagues who are partnering with me in BoD research through the European Burden of Disease Network). As the country lead on the GBD Study, I have co-authored and contributed to highly cited impactful peer-reviewed publications in the Lancet, Nature, Nature Medicine and BMJ since 2016. My global collaborative research work has led to two successful European grants: the EU COST Action on the European Burden of Disease Network (2019-2024) and the Horizon 2020 grant- the unCoVer project to lead on ethics and data protection across 29 EU and non-EU partners during the Covid-19 pandemic (2020-2023). As the country lead on the EU COST Action project and serving on the management committee, I have contributed to research, capacity building and knowledge translation. I was the lead organizer of a Knowledge Translation workshop in UCC in April 2023 hosting 25+ EU COST Action partner country delegates, including representatives from WHO, HSE and the Irish Department of Health. The unCoVer project established a federated open science platform on real-world-data, using Opal- DataSHIELD. I also led the work package on population health modelling of the HRB Centre for Health & Diet Research employing the GBD methodology from 2014 to 2020 resulting in successful completion of the first PhD Scholar on GBD methodology in Ireland (Dr Shelly Chakraborty).
As the GBD Study country lead, my citation metrics have improved significantly- being the 2nd most cited Irish researcher since 2019 (>87,000 citations since 2019; >12,000 citations in 2024; h-index: 60; total citations >100,000; 200+ peer-reviewed publications), and thus have contributed to UCC international rankings contemporaneously. My GBD-related work featured in the Sunday Times UK in 2023 and on the IHME website. I have extensively contributed to GBD capstone papers in the Lancet since 2016, and to my two core research areas: tobacco control and alcohol policy. I am now a regular external peer reviewer of GBD-related manuscripts submitted to the Lancet series, and was invited for a commentary to the Lancet Infectious Disease in 2022.
Other current research contributions (2021- onwards) and its ongoing impact:
1) Alcohol Policy Research I have two consecutive successful IRC (Irish Research Council) New Foundation grants (2021/2022) to work on alcohol policy research in collaboration with Alcohol Action Ireland (AAI)- the national advocate and campaigner for reducing alcohol harm in Ireland- as a civic partner. In the first IRC project I estimated the burden of alcohol attributable deaths in Ireland employing the GBD methodology, which led to an important policy paper. This work estimating 4 deaths every day due to alcohol consumption in Ireland is informing alcohol policy in Ireland, cited by policy makers. The same work has been referenced in the House of the Oireachtas on the Sale of Alcohol Bill 2022. This GBD work featured in the Irish Examiner. I was invited as a guest speaker to the first-ever Alcohol Policy Research Conference in Ireland co-hosted by HRB and the Irish Department of Health in September 2023.
The second IRC project evaluated the structural separation compliance article of the most comprehensive Public Health Alcohol Act, 2018 of Ireland. The work featured in the AAI Annual Review Report 2022. Outputs from both these IRC-funded projects have been instrumental in public health advocacy shaping the national alcohol policy in Ireland. First, I was one of the signatories of an open letter re-assessing the recent Sale of Alcohol Bill 2022, which drew media attention. Secondly, two online webinars- “Harnessing our health data” (February 2022) and “Out of balance?” (December 2022) were organized for wider broadcasting to both researchers and lay audience alike. I have been invited to serve as a key member on the recently established alcohol policy research group of the HSE Office of National Health Improvement led by Dr Diarmuid O’ Donovan and co-led by Dr Aisling Sheehan. My work on alcohol policy research contributed to an invited piece in the Think Global Health- an initiative from Council on Foreign Relations, US. I will be working on the Burden of Proof methodology with GBD Alcohol Modelling Team.
Currently, I am on the Advisory Board and a mentor of a recently 4-year funded HRB AARP project (2023-2027) to examine the Public Health (Alcohol) Act, 2018 of Ireland in-depth- involving a mixed-methods study led by Dr Susan Calnan of School of Public Health, UCC and external partners from Scotland and the Alcohol Action Ireland. I am also supervising a master’s project on examining the trends in alcohol-attributable deaths, hospitalizations and consumption patterns in Ireland pre-minimum unit pricing (MUP) legislation in Jan 2022 in collaboration with the Belgian Public Health Institute- Sciensano, Brussels.
2) Migration and Tuberculosis I am co-supervising a PhD project work of Sarah Jackson – a senior epidemiologist at HPSC, with Professor Catherine Comiskey of TCD on the “Effect of migration on the epidemiology of TB in Ireland”. This work is contributing to the National TB strategy in Ireland, for instance, the rethinking of selective BCG vaccination to high-risk individuals in Ireland. This work featured in the Irish Independent in 2024. This builds on my previous and current work on WHO End TB Strategy using GBD data.
3) Mental Public Health and Chronic Disease (Cancer) I am the primary supervisor of a 4-year HRB-funded MHAINTAIN scholar (2022-2026)- Doireann Ni Dhalaigh – a mixed methods study examining the early identification of risk factors of suicidal ideation in newly diagnosed cancer patients, with colleagues from HSE and CUH- Professor Eugene Cassidy and Dr Fahmi Ismail. A scoping review of the methods of suicide by people with cancer has been recently published in HRB Open Research. This is a collaborative study with the National Suicide Research Foundation. This builds on my recent work on systematically analyzing public mental health conditions during the Covid-19 pandemic- a) mental health among health care workers; b) suicide rates before and during the pandemic. One of the objectives of this project is a linkage analysis- employing the National Cancer Registry of Ireland (NCRI) data – linking cancer incidence with self-harm/mortality data of National Self-Harm Registry and CSO, respectively to build capacity in the methodology of linkage analysis on the island of Ireland.
4) Estimating the Burden of Covid-19 and Long Covid in Ireland & Health Literacy I am currently supervising a MD Scholar at the School of Public Health- Dr Tochukwu Kingsley. We are applying and validating the newly developed Covid-19 Burden of Disease estimation methodology to the Irish data for the first 2 years of the pandemic. We are estimating the burden of Long Covid-19 applying GBD Long Covid-19 methodology for the first time in Ireland. This work builds on our previous work of estimating the burden (Disability-Adjusted-Life-Years) of Covid-19 pandemic for the first full year in Ireland in collaboration with the EU COST Action Covid 19 Task Force. I am on the Advisory Board of a related Covid-19 PhD work led by Julie Arnott at the School examining health literacy, infodemic during the Covid-19 pandemic. This builds on my previous work on oral health literacy study, and readability of public health websites during pandemic.
5) Oral Health Burden Study I co-supervised a SURE awardee at the School of Dentistry in UCC- Sehrish Mahmood, with colleagues from the Oral Health Services Research Centre, UCC- Professor Mairead Harding on examining the impact of sugar-sweetened beverage consumption patterns on oral health in Ireland using the GBD 2019 Study. This study was presented at the BASCD (British Association for the Study of Community Dentistry) summer scientific meeting 2023 in UCC, and at the IADR 2023 conference. I was invited as a guest speaker at this BASCD conference 2023 on oral health and tobacco use, and also was a panelist. I have advanced Dental Public Health discipline supervising MDPH (Master of Dental Public Health) students- latest being exploring the burden of lip, oral and pharyngeal cancer across EU, using the GBD 2019 study by Dr Aisling O Sullivan. The same study was presented at BASCD 2022 conference. This study builds on an earlier work on oral cancer incidence trends in Ireland in collaboration with the National Cancer Registry of Ireland (NCRI) team.
6) Ageing & Frailty Index- a methodological approach I co-led on a methodological paper developing and validating a population-based frailty index using the GBD data – the GBD-FI Study in collaboration with EU COST Action team, Mercy Hospital, UCC (Professor Ronan O’ Caoimh) and PhD scholar- Mark O’ Donovan. This index was later externally validated using SHARE (Survey of Health Ageing and Retirement in Europe) data in 2023.
7) Obesity and Health Impact across the lifecourse I postulated the hypothesis of an potential epidemiologic link between the rise in adenocarcinoma and obesity in 2003, and commented on the histologic subtypes of lung cancer incidence and obesity during my PhD project. I provided additional insights into a lifecourse approach to adult disease and childhood obesity in low and middle income nations. Such observations led to further advancing this sub-discipline through successive studies in India, co-supervising a PhD scholar in India – Dr DS Prasad. I further explored the lifecourse pathway using large Irish cohorts to identify if post-natal secondhand smoke exposure increases the risk of childhood obesity, which featured in the Irish Examiner. More recently, we showed that increased adiposity is associated with mental health among middle-age population, which featured in the ABC news in 2024. Further explorations employing the GBD data are in progress.
8) Life Expectancy, Healthy Life Expectancy, Health Care Access & Universal Health Coverage: The Healthy Ireland Framework has Healthy Life Expectancy (HLE) as an indicator for two of the Goals- to increase HLE (Goal 1) and to reduce the gap in HLE (Goal 2).

Secondly, two online webinars- “Harnessing our health data” (February 2022) and “Out of balance?” (December 2022) were organized for wider broadcasting to both researchers and lay audience alike. I have been invited to serve as a key member on the recently established alcohol policy research group of the HSE Office of National Health Improvement led by Dr Diarmuid O’ Donovan and co-led by Dr Aisling Sheehan. My work on alcohol policy research contributed to an invited piece in the Think Global Health- an initiative from Council on Foreign Relations, US. I will be working on the Burden of Proof methodology with GBD Alcohol Modelling Team.
Currently, I am on the Advisory Board and a mentor of a recently 4-year funded HRB AARP project (2023-2027) to examine the Public Health (Alcohol) Act, 2018 of Ireland in-depth- involving a mixed-methods study led by Dr Susan Calnan of School of Public Health, UCC and external partners from Scotland and the Alcohol Action Ireland. I am also supervising a master’s project on examining the trends in alcohol-attributable deaths, hospitalizations and consumption patterns in Ireland pre-minimum unit pricing (MUP) legislation in Jan 2022 in collaboration with the Belgian Public Health Institute- Sciensano, Brussels.
2) Migration and Tuberculosis I am co-supervising a PhD project work of Sarah Jackson – a senior epidemiologist at HPSC, with Professor Catherine Comiskey of TCD on the “Effect of migration on the epidemiology of TB in Ireland”. This work is contributing to the National TB strategy in Ireland, for instance, the rethinking of selective BCG vaccination to high-risk individuals in Ireland. This work featured in the Irish Independent in 2024. This builds on my previous and current work on WHO End TB Strategy using GBD data.
3) Mental Public Health and Chronic Disease (Cancer) I am the primary supervisor of a 4-year HRB-funded MHAINTAIN scholar (2022-2026)- Doireann Ni Dhalaigh – a mixed methods study examining the early identification of risk factors of suicidal ideation in newly diagnosed cancer patients, with colleagues from HSE and CUH- Professor Eugene Cassidy and Dr Fahmi Ismail. A scoping review of the methods of suicide by people with cancer has been recently published in HRB Open Research. This is a collaborative study with the National Suicide Research Foundation. This builds on my recent work on systematically analyzing public mental health conditions during the Covid-19 pandemic- a) mental health among health care workers; b) suicide rates before and during the pandemic. One of the objectives of this project is a linkage analysis- employing the National Cancer Registry of Ireland (NCRI) data – linking cancer incidence with self-harm/mortality data of National Self-Harm Registry and CSO, respectively to build capacity in the methodology of linkage analysis on the island of Ireland.
4) Estimating the Burden of Covid-19 and Long Covid in Ireland & Health Literacy I am currently supervising a MD Scholar at the School of Public Health- Dr Tochukwu Kingsley. We are applying and validating the newly developed Covid-19 Burden of Disease estimation methodology to the Irish data for the first 2 years of the pandemic. We are estimating the burden of Long Covid-19 applying GBD Long Covid-19 methodology for the first time in Ireland. This work builds on our previous work of estimating the burden (Disability-Adjusted-Life-Years) of Covid-19 pandemic for the first full year in Ireland in collaboration with the EU COST Action Covid 19 Task Force. I am on the Advisory Board of a related Covid-19 PhD work led by Julie Arnott at the School examining health literacy, infodemic during the Covid-19 pandemic. This builds on my previous work on oral health literacy study, and readability of public health websites during pandemic.
5) Oral Health Burden Study I co-supervised a SURE awardee at the School of Dentistry in UCC- Sehrish Mahmood, with colleagues from the Oral Health Services Research Centre, UCC- Professor Mairead Harding on examining the impact of sugar-sweetened beverage consumption patterns on oral health in Ireland using the GBD 2019 Study. This study was presented at the BASCD (British Association for the Study of Community Dentistry) summer scientific meeting 2023 in UCC, and at the IADR 2023 conference. I was invited as a guest speaker at this BASCD conference 2023 on oral health and tobacco use, and also was a panelist. I have advanced Dental Public Health discipline supervising MDPH (Master of Dental Public Health) students- latest being exploring the burden of lip, oral and pharyngeal cancer across EU, using the GBD 2019 study by Dr Aisling O Sullivan. The same study was presented at BASCD 2022 conference. This study builds on an earlier work on oral cancer incidence trends in Ireland in collaboration with the National Cancer Registry of Ireland (NCRI) team.
6) Ageing & Frailty Index- a methodological approach I co-led on a methodological paper developing and validating a population-based frailty index using the GBD data – the GBD-FI Study in collaboration with EU COST Action team, Mercy Hospital, UCC (Professor Ronan O’ Caoimh) and PhD scholar- Mark O’ Donovan. This index was later externally validated using SHARE (Survey of Health Ageing and Retirement in Europe) data in 2023.
7) Obesity and Health Impact across the lifecourse I postulated the hypothesis of an potential epidemiologic link between the rise in adenocarcinoma and obesity in 2003, and commented on the histologic subtypes of lung cancer incidence and obesity during my PhD project. I provided additional insights into a lifecourse approach to adult disease and childhood obesity in low and middle income nations. Such observations led to further advancing this sub-discipline through successive studies in India, co-supervising a PhD scholar in India – Dr DS Prasad. I further explored the lifecourse pathway using large Irish cohorts to identify if post-natal secondhand smoke exposure increases the risk of childhood obesity, which featured in the Irish Examiner. More recently, we showed that increased adiposity is associated with mental health among middle-age population, which featured in the ABC news in 2024. Further explorations employing the GBD data are in progress.
8) Life Expectancy, Healthy Life Expectancy, Health Care Access & Universal Health Coverage: The Healthy Ireland Framework has Healthy Life Expectancy (HLE) as an indicator for two of the Goals- to increase HLE (Goal 1) and to reduce the gap in HLE (Goal 2).
